What to include with a referral
- Patient name and preferred contact details
- Reason for referral and the tooth or region involved
- Recent radiographs, if available (panoramic or CBCT)
- Relevant medical history and current medications
- Any time sensitivity or specific concern to flag

- A prompt call to schedule the consultation
- A clear explanation of the procedure and anesthesia options
- Written pre-op and post-op instructions
- A return to your care with a summary of treatment

Patient information sent with a referral is protected health information. Our referral portal is hosted by a HIPAA-compliant partner under a signed Business Associate Agreement, so referral details never pass through this website. When a portal is not yet connected, please call or fax the office and we will guide you.
